I was working on a document creation task, and the system output 3 quite different files instead of 1. When I asked the chat why it did this, it denied it and said that it only created 1 file. I have seen this happen twice now, and I noticed that each output file contains different material. Not only is this creating extra work for me to read each version to find the "best" format, or piece them together, but I am worried that it is tripling our output tokens.
